Thanks to the generosity of so many, we are now able to provide remembrance plates free of charge to anyone in the US who did not receive a plate and has experienced infant or child loss. In order to provide this service to more families, we ask that you only request one plate per loss.

 

If you would like additional plates, those are available by making a donation online. Our goal is to grow Mamie’s nationwide to reach all families who experience infant or child loss. If your local hospital is not fully funded, we encourage you to spread the word about Mamie’s in your community.

 

 Print the form and mail it to us completed with a copy of your child's prints. Please allow 8-10 weeks for your plate to be finished.
